Please have the following items ready for your appointment:
  • Document needing to be notarized already filled out, except for the final signature
  • Valid, unexpired government-issued identification (State Driver's License, State ID, US Passport, military ID, etc.)
    • If the above IDs are not available, we will need to have someone act as a credible witness.  The credible witness is placed under oath to swear/affirm the signer's identity.  This person must personally know the signer and have one of the above forms of identification.  In addition, the credible witness cannot be connected to the notarized transaction.  
  • You! The individual signing and requesting notarization is required by Arizona law to appear before the public notary at time of notarization

For a remote online notarization:
  • Electronic version of the document.  Please upload the document via this page.  Fill out the form and upload your document through this form.  If you do not have an electronic version of your document, you can scan the paper copy with a scanner or your smartphone.
  • After the document has been received, an invoice will be sent to the total price for the notarization.
  • A link will be sent to your provided email to enter a secure Zoom meeting.  At this time, you will be asked to use your smartphone to take pictures of your ID (either a driver's license, state ID, or US Passport).  Next you will be asked questions to verify your identity.
  • Once your ID has been validated and you have answered the authentication questions correctly, the Zoom meeting will begin.  At this time you will electronically sign the document and then we will electronically notarize and embed security features into the document.
  • After this you will receive a link in your email to the signed and notarized document for you to download and print.
